Dr. Min Cole, Ph.D. in Applied Gerontology, is the Founder of Continuous Care Inc. With over 25 years of leadership across aging and long-term care, she specializes in person-centered program design, caregiver education, and quality management. Dr. Cole also founded the 501(c)(3) Quality of Life Group, which delivers case management, caregiver support, and resource navigation for older adults and their families.
